New Patient Registration
In order to register with the Practice you should bring your NHS medical card together with ID showing your current address. If you don’t have a medical card the Receptionist will ask you to complete a form.
You will be invited to a registration medical appointment with the Practice Nurse.
Practice Area
The Practice serves the area between the boundaries of, but excluding North Berwick and Longniddry. Due to space restrictions we cannot accept new patients from North Berwick or Longniddry.

You can download the PDF version of our registration form below. Fill it out and bring it to the surgery together with ID showing your new address. This process needs to be completed before you need to see a doctor or nurse so please make sure we have the forms as soon as possible. We would also like to know if you are a carer and whom you care for, or if you are someone who is being cared for then let us know who your carer is.
